1- نموذج طلب تسجيل الحليب المصنّع محلياً ( لعمر ما فوق 3 سنوات) وأغذية الأطفال MPH341-2معبأ حسب الأصول وموقّع من المسؤول عن المؤسسة. 2- ملف التعريف عن المصنع أو صاحب إجازة التسويق مصدق حسب الأصول. 3- شهادة تصنيع جيّد مصدّقة حسب الأصول. 4- إفادة من الشركة الصانعة عن محتويات المستحضر. 5- في حال إختلاف في التركيبة و/أو الإسم التجاري يجب تقديم كتاب صادر عن الشركة الصانعة يشرح سبب الإختلاف ويبيّن تفاصيله. 6- طرق تحليل المستحضر. 7- معلومات مفصلة عن ثبات الصنف Stability data على طبخة واحدة. 8- مواصفات المنتج النهائي ((Finished Product Specifications 9- نماذج بيع أصلية عدد2. 10- شهادة تحليل أصلية للنماذج المقدمة من أحد المختبرات المرجعية المعتمدة في وزارة الصحة العامة. |
